How to Enter

Entry into the drawing must be completed at shotatamillion.ky.gov. Entry requires providing your name, birthdate, email address, phone number, home address and the name and location of the place that provided you with the vaccine. Entry also requires agreement to the official rules of the drawing and agreement to allow the Kentucky Department for Public Health to verify your vaccination information.

Entry for children between the ages of 12 and 17 further requires a parent or guardian’s name, email address and phone number.

If you do not have access to the internet, you can call 855-598-2246 to provide information to enter the drawings.

Kentuckians 18 years old and older who have received at least their first dose of a Moderna or Pfizer COVID-19 vaccine, or the one-dose Johnson & Johnson vaccine, may enter to win one of three $1 million drawings.

Kentuckians 12 to 17 years old who have received at least their first dose of the Pfizer COVID-19 vaccine may enter to win one of 15 full scholarships to a Kentucky public college, university, technical or trade school, which includes tuition, room-and-board and books.

Eligibility and Verification

Permanent residents of Kentucky are eligible to enter the Team Kentucky Shot At A Million Drawing if:

  • Legal resident of the U.S. and a resident of Kentucky;
  • At least 12 years of age;
  • For 12 year olds, a parent or guardian must have completed the application in order to be eligible – in terms;
  • Not incarcerated for a felony conviction;
  • Not an employee of Office of the Governor, Cabinet for Health and Family Services, the Kentucky Higher Education Assistance Authority, and Kentucky Interactive, LLC or blood relative or spouse of such employees or anyone living in the same household of such employees.
  • Have received at least the first dose of Pfizer or Moderna COVID-19 vaccine or one dose of Johnson & Johnson COVID-19 vaccine.

To win, the entrant’s eligibility must be verified by Kentucky Department for Public Health officials.

General Rules

The potential winners of the drawings will be randomly selected from among all eligible entries received and subject to verification of eligibility and compliance with the Rules. Upon the random selection of an entrant and verification, the entrant will be declared the official winner of the drawing.

Eligible Kentuckians can only submit one (1) entry for the drawings. Prior to each drawing, all duplicate entries will be removed.

Eligible entrants age 18 and Over will be eligible for a random drawing for one (1) $1 million cash prize per drawing

Eligible entrants ages 12 to 17 will be eligible for one (1) full scholarship to a Kentucky public state college or university or technical or trade school.

All winners will be removed from future drawings.

All entries received by the deadline will be included in all future drawings for the Shot at a Million promotion.

Prizes

For eligible entrants age 18 and over: One (1) $1 million cash prize per drawing

For eligible entrants ages 12 to 17: One (1) full scholarship to a Kentucky public state college or university or technical or trade school. The scholarship will be in the amount of the qualifying expenses for in-state tuition, books and room-and-board for a four-year program, two-year program, community college program, or technical or trade school. The funds are available until the winner reaches the age of 28, after which any remaining funds will revert to the state.

All prize details shall be determined in the sole and absolute discretion of the Commonwealth of Kentucky. Kentuckians who do not meet the eligibility requirements defined herein are not eligible to win.

Prizes are nontransferable to any other person, including friends or relatives. Fulfillment of the cash prize will occur approximately six (6) weeks after the announcement date. Fulfillment of the scholarship fund prize will occur at the time of enrollment in a qualifying secondary educational institution.

Alternate Winners

If a potential winner cannot be verified, violates the Official Rules, refuses to accept the prize, or cannot be located, an alternate potential winner shall be drawn and declared the official winner upon verification.

The promptness and degree of reasonableness of efforts to locate a potential winner or any decision to declare a potential winner ineligible under these Rules shall be at the sole discretion of the Commonwealth of Kentucky.

Alternates will be used in the order in which they were randomly selected as vacated spots become available.

Potential Winners

Any information entered for or by a minor child or adult under a guardianship must be validated by a parent or legal guardian prior to award of a prize. Any entrant subject to this condition must include the name, address, phone number and email address of the parent or legal guardian.

The Department for Public Health reserves the right to require presentation of information for verification prior to the award of any prize.

By entering the promotion, all entrants authorize the Commonwealth of Kentucky to access vaccination records to verify that a COVID-19 vaccination has been received.

Tax Implications

Official winners will be responsible for any taxes associated with the prizes. Federal, state and local taxes are the sole responsibility of the official winners.

For the $1 million cash prize, an IRS Form 1099 will be issued by the Commonwealth of Kentucky to each winner. All federal, state and local laws, applicable taxes and regulations apply.

For the scholarship fund prize, the funds will be distributed per agreement between the Kentucky Department for Public Health and the Council for Postsecondary Education into the Kentucky 529 Program. The program will accept the funds and place them into a scholarship account on behalf of each winner. The program will manage the funds for the recipients as beneficiaries. As structured, the scholarship account to be used for qualifying expenses for in-state tuition, books and room-and-board does not incur tax liability to the beneficiaries, based upon current federal and state law, which is subject to change. 

The official winner should consult an independent tax advisor regarding application of all applicable tax laws.

Other Conditions

The cash or scholarship prizes may be reduced by delinquent monies owed to the Commonwealth of Kentucky, if applicable.

Acceptance of the prize constitutes express permission for the Commonwealth of Kentucky and its agencies as set forth in the Release and Verification to use the prize winner’s name and/or likeness worldwide, in perpetuity, for purposes of advertising and trade without further compensation, notification or permission unless prohibited by law, in any and all media now known or hereafter devised.

Acceptance of the prize constitutes agreement to hold the Commonwealth of Kentucky, any of its employees, affiliates, subsidiaries, contractors, assignees and agents absolutely harmless against any and all claims and/or liability arising out of the ownership and/or use of a prize. Each prize winner assumes all liability for any injury or damage caused, or claimed to be caused, by participation in the Shot at a Million Promotion or use or redemption of any prize.

The Commonwealth of Kentucky makes no warranty, guarantee or representation of any kind concerning any prize and disclaims any implied warrant.

By participating in the Shot At A Million Drawing, all entrants agree to be bound by the Official Rules.

These Official Rules are subject to review and may be amended or revised at any time at the sole discretion of the Kentucky Department for Public Health.

The Commonwealth of Kentucky reserves the right, at its discretion, to change the prize(s) to be awarded as part of this Promotion or end the Promotion prior to the completion of all scheduled drawings.

The Commonwealth of Kentucky and any of its employees, affiliates, subsidiaries, contractors, assignees or agents are not responsible for late, or incomplete or misdirected entries, or for incorrect or inaccurate information received, whether caused by the website, or any of the equipment or programming associated with or utilized in this promotion.

The Commonwealth of Kentucky and any of its employees, affiliates, subsidiaries, contractors, assignees or agents are not responsible for any error, omission, interruption, deletion, defect, delay in operation or transmission, communication failure, regardless of the cause, with regard to any equipment, systems, networks, lines, satellites, servers, computers or providers utilized with this promotion, which may include limitation or inaccessibility of the internet, this website, or any combination thereof, theft or destruction or any unauthorized access or use of this website, or any injury or damage to any device related to or resulting from participating in this promotion or downloading materials from or use of this website.

Any attempt to deliberately damage the promotion website or undermine the legitimate operation of this promotion or disrupt the purpose of this website is a violation of criminal and civil laws.
